Recently, our team of health and safety professionals conducted three very different incident investigations:
- A vehicle collision involving a public agency employee
- An operational incident resulting in worker injuries
- A fatal event involving hydrogen sulfide (H₂S) exposure
Although each incident had distinct circumstances, the organizational risk factors they revealed were strikingly similar.
Case Study 1: Vehicle Collision – No Organizational Root Cause Identified
In this case, a municipal employee driving an official vehicle was struck by a bicyclist. Through interviews, operational review, and evaluation of official police reports, it was determined that:
- The employee was trained, licensed, and acting appropriately
- No Cal/OSHA or California Vehicle Code regulations were violated
- Environmental and operational conditions were normal
- The bicyclist’s actions – not organizational practices – precipitated the collision
The analysis found no organizational root cause within the employer’s control.
However, the investigation revealed non-contributing but meaningful observations such as unclear daily vehicle inspection criteria, missing documentation, and assumptions that checks had been performed by prior drivers. These issues did not cause the incident but correcting them strengthens safety systems and improves regulatory compliance management.
Case Study 2: Operational Safety Incident – Multiple Contributing Factors
In a separate incident at a commercial facility, a worker injury prompted an in-depth analysis. Our investigation revealed multiple contributing factors:
- Established procedures existed but were not consistently executed
- Certain hazards were not fully evaluated
- Supervisors and employees had different interpretations of expectations
- Training had been completed, but retention varied by experience level
- Controls were available but not universally applied
This is a great example of process drift where small inconsistencies accumulate into significant risk. This scenario underscores why workplace safety audits and ongoing coaching are essential to prevent exposure to occupational safety hazards.
Case Study 3: Fatal Hydrogen Sulfide Incident – Missed Hazards with Lethal Consequences
The most critical case involved a fatality at a resort where an employee collapsed while entering an underground vault. Our industrial hygiene team conducted atmospheric monitoring using calibrated multigas instruments.
Our industrial hygiene consultants documented the following:
- H₂S levels at the opening ranging up to 4.4 ppm
- H₂S concentrations inside the vault peaking at 67.5 ppm after continuous purging for at least one full day
- Volatile Organic Compounds (VOCs) levels peaking at 16.2 ppm
These measurements exceeded multiple Cal/OSHA exposure limits, indicating the likely presence of an immediately dangerous atmosphere.
Contributing factors included:
- Standing water enabling slow, insidious gas accumulation
- No atmospheric testing prior to entry
- Lack of prior incident investigation or corrective action
We recommended immediate steps, including:
- Hazard purging and retesting
- Formal incident investigation per CCR §3203
- Establishing compliant confined space entry procedures
This case illustrates the importance of exposure assessments, hazard anticipation, and strict procedural compliance.
What These Incidents Teach Us About Organizational Risk
Across industries, incident investigations consistently reinforce five concepts:
- Not all incidents have an organizational root cause – but every incident has lessons.
Even when an organization is not at fault, meaningful opportunities for continuous improvement still emerge.
- Small process drift can create vulnerability.
Unclear criteria, assumptions, training gaps, and inconsistent enforcement can accumulate significant risk over time.
- Hazards must be continuously evaluated.
In high-risk environments such as a confined space, conditions can change rapidly. Hydrogen sulfide (H₂S), Volatile Organic Compounds and other contaminants can develop or intensify quickly. Ongoing air monitoring, air sampling, and exposure assessments are critical.
- Documentation matters.
Regulatory requirements emphasize not only program development but also consistent implementation, verification, and recordkeeping.
- Objective investigations support stronger compliance.
Third-party investigations often reveal issues internal teams overlook, providing defensible findings that strengthen regulatory compliance and reduce future liability.
